Can you tattoo over a mole or a birthmark?
No, and no reputable studio will. We design around moles and birthmarks rather than covering them, because a mole needs to stay visible so that you and a doctor can see if it ever changes.
Why not
Changes in a mole are how skin cancer is usually spotted, and ink over the top hides exactly the thing anybody would be looking for. It can also make a mole harder to assess later, or the pigment can obscure a biopsy.
This is a firm rule rather than a preference, and it does not change because the mole is small or because it is in the middle of the design.
What we do instead
Design around it, with a small margin of clear skin. In busy work a mole usually disappears into the composition without anybody noticing it was worked around.
For freckles the same principle applies loosely, though a light scatter of freckles under fine work is not the same concern as a raised mole.
If a mole has changed
See a GP before you book, not afterwards. Changing shape, changing colour, bleeding, itching or getting bigger are all reasons to have it looked at, and it is one appointment.

It is safe in a studio that is registered, uses single-use needles and opens them in front of you. Every tattooist and premises in England and Wales must be registered with the local council, and you are entitled to ask to see the certificate.

Usually, once the scar is fully mature, which normally means at least a year and sometimes two. Scar tissue takes ink differently from the skin around it, so the design has to be planned for that rather than dropped on top of it.

Turn up rested, fed and hydrated, and do not drink alcohol beforehand. Good preparation genuinely reduces how much it hurts, cuts down bleeding and helps the skin take the ink cleanly.
